Enhanced Care Management (ECM) – FAQs

What is Enhanced Care Management (ECM)?

ECM is a CalAIM service that provides whole-person care coordination for members with complex medical and social needs. Members are connected with a dedicated in-home care services manager who helps coordinate healthcare, behavioral health, and community-based in-home care services.

Who may be eligible for ECM services?

Members may qualify for ECM if they are:

  • Individuals at risk for avoidable hospital or emergency department utilization
  • Adults living in the community and at risk for long-term care (LTC) institutionalization
  • Adult nursing facility residents transitioning back into the community

Eligibility is determined by the Medi-Cal health plan.

How do I refer someone (or myself) for ECM?

Please complete the referral form below. Our in-home care services team will take it from there and keep you informed of the referral status.

How does ECM get approved?

An in-home care services member must receive authorization from their Medi-Cal health plan.

  • Referrals may be made by a primary care provider, hospital, community organization, county agency, or the member themselves.
  • The health plan reviews eligibility and medical necessity.
  • If approved, the member is assigned an in-home care services manager and services begin.

How long does it take to get ECM in-home care services started?

Timeframes vary:

  • Urgent referrals may be expedited
  • Standard referrals typically take 10–20 days

⏳ Approval timelines are not guaranteed and depend on the health plan.

Can a member receive ECM services while on hospice?

No. ECM services cannot be provided while a member is on hospice, as this would be considered a duplication of services.

What happens if a member moves to a different county or changes Medi-Cal plans?

ECM services do not automatically transfer to a new county or health plan. Services must be reauthorized through the member’s new Medi-Cal health plan.

Community Supports – FAQs

What Community Supports does J&M Homecare provide?

J&M Homecare offers the following CalAIM Community Supports:

  • Personal Care Services
  • Homemaker Services
  • Respite Services

These in-home care services do not replace IHSS.

Can I keep IHSS if I receive Community Supports?

Yes! Members may keep IHSS while receiving Community Supports. However, IHSS providers and J&M Homecare caregivers cannot work the same shifts.

Will Community Supports reduce or change my IHSS hours?

No. Participation in Personal Care, Homemaker, or Respite Services does not affect IHSS hours. These are separate programs.

Who is eligible for Personal Care and in-home care services?

Members may be eligible if:

  • Their IHSS application is still pending
  • They have requested an IHSS reassessment or redetermination
  • They already receive maximum IHSS hours, but still need additional support

Who is eligible for Medi-Cal Respite Services?

Members may be eligible if they:

  • Have a caregiver (IHSS provider, family member, friend, neighbor, etc.), and
  • Need respite services to help prevent caregiver burnout

Can Community Supports hours be renewed or increased?

Respite Services

  • Approved for up to 336 hours per calendar year
  • Renewals may be requested for the following year
  • Exceptions may be considered for approval of hours beyond 336 if there is a change in the caregiver’s situation (e.g., medical treatment or hospitalization)

Personal Care & in-home care services Services

  • Hours vary based on member need
  • Extension requests may be submitted (approval not guaranteed) if:
  • The member’s IHSS application is still pending
  • An IHSS reassessment has been requested
  • The member already receives maximum IHSS hours, but still needs help

Can J&M Homecare caregivers provide transportation?

No. Medi-Cal does not cover transportation while members are receiving Personal Care, Homemaker, or Respite Services.

🚗 Members should use the free transportation services provided by their Medi-Cal health plan for medical appointments. Or, Transportation Service can be purchased separately

How do I know if I’m eligible for Community Supports through J&M Homecare?

  • You must have active Medi-Cal, and
  • J&M Homecare must be contracted with your Medi-Cal health plan
