Enhanced Care Management

J&M Homecare has partnered with many different California Managed Care Plans to help support and transform Medi-Cal through the CalAIM program.

What is CalAIM?

CalAIM stands for California Advancing and Innovating Medi-Cal.

Medi-Cal members will have access to new and improved services to get well-rounded care that goes beyond the doctor’s office or hospital and addresses all of their physical and mental health needs.

These changes are part of a broad transformation of Medi-Cal to create a more coordinated, person-centered, and equitable health system that works for all Californians.

Enhanced Care Management is a statewide Medi-Cal benefit available to members with complex needs. Enrolled Medi-Cal members receive support from a single lead care manager who assists in coordinating their health and health-related care, including physical, mental, dental care, and social services. Enhanced Care Management breaks down the traditional walls of health care to extend care into members’ communities and introduces a better way to coordinate care.

Enhanced Care Management with J&M Homecare is a 100 percent covered benefit so there is no cost or co-pays due from enrolled members.

About the Program:

According to the American Association of Retired Persons AARP, 77% of individuals desire to be at home for their Long-term care.  However, aging can present surprises and oftentimes we don’t know what we don’t know. Having the support of a Care Coordinator helps reduce emergencies, hospital admissions, and improves health outcomes for the members we serve.

With the compassionate and expert guidance of a care manager, we work together to optimize your or your loved ones’ aging experience in alignment with your goals and desires.
