Exciting News! J&M Homecare is now a Medi-Cal provider under the CalAIM initiative, allowing eligible members to receive our professional in-home health care services at no cost as a covered benefit. This program is designed to improve access to coordinated, high-quality care that supports individuals and families in maintaining health, independence, and stability at home.

We are currently contracted with Kaiser Permanente, Partnership Health Plan, and Contra Costa Health Plan, and we are actively working to expand our network through additional partnerships. CalAIM (California Advancing and Innovating Medi-Cal) is a multi-year initiative led by the California Department of Health Care Services (DHCS) that is transforming and strengthening the Medi-Cal program—California’s version of Medicaid. The goal of CalAIM is to improve access to quality care by focusing on whole-person, coordinated health services that extend beyond traditional medical treatment.

Launched in January 2022, CalAIM was designed to make Medi-Cal more person-centered, equitable, and responsive, particularly for individuals and families with complex medical, behavioral, and social needs. This approach allows eligible members to receive meaningful support services directly in the home and community.

CalAIM:

  • Expands services beyond traditional healthcare (e.g., housing supports, meal delivery, home modifications)
  • Integrates physical health, behavioral health, and social services

As part of this innovative program, we offer coordinated, person-centered services designed to support Medi-Cal members with complex health and social needs. Enhanced Care Management

Enhanced Care Management (ECM) is one of the Medi-Cal featured programs J&M Homecare can now offer for free.

Enhanced Care Management (ECM) services aim to address both clinical and non-clinical needs to improve overall health outcomes, reduce unnecessary hospital use, and support long-term well-being.

At J&M Homecare, as a part of ECM, we offer:

 

Whole-Person, Person-Centered Care

  • ECM focuses on the entire person, not just medical issues.
    • Physical health
    • Mental and emotional health
    • Social needs
    • Cultural and spiritual values
  • The person helps create and shape their care plan
  • Care is tailored to individual goals, preferences, and cultural values.
  • Every interaction fosters a sense of being heard, understood, and cared for.

Care Coordination

  • Our ECM Care Managers work with the member to help coordinate care with their doctors, specialists, hospitals, and caregivers.
  • Our Care Managers work with multidisciplinary team members to ensure members are receiving appropriate medications, therapies, and other care needs.
  • Our Care Managers help ensure members are attending appointments, following recommendations, and assisting with transitional care services (e.g., from hospital to home).
  • Our Care Managers oversee all parts of a person’s care (medical, behavioral, dental, social, housing, etc.)

Community-Based

  • Services are often delivered in homes or in other community settings, not just in clinical settings or hospitals.
  • Enhanced Care Management prioritizes building relationships and helps to address barriers to care, such as transportation, housing, food insecurity, or language.

Dedicated Care Team

  • Each member is assigned a Lead Care Manager to help build their care plan and navigate additional services.
  • J&M Homecare has a robust, dedicated team for Medi-Cal members’ services.

Community Supports Services

Community Supports are cost-effective, medically appropriate services that address social determinants of health. One of the key Community Supports is Home Care Services, designed to help people remain safely in their private homes or communities, wherever they call home.

Helping individuals who have difficulty managing Activities of Daily Living tasks at home, with the ultimate goal of assisting them so that they can live safely and independently at home.

At J&M Homecare, individuals are assigned a single Care Services Manager who will work with the member and their chosen support system to coordinate care services.

Examples of Home Care Services:

At J&M Homecare, as a part of Medi-Cal’s Community Support Services, we offer:

Personal Care Services & Homemaker Services:

  • Helps individuals with ADL’s (activities of daily living) that they can no longer manage on their own due to limitations or health challenges.
  • They are delivered in the home or their current living community, and focus on maintaining independence, safety, and dignity.
  • Support members with activities of daily living, such as bathing, dressing, meal prep, and helping the recipient stay safe and independent, wherever they call home.
  • Assist members with light housekeeping to maintain a healthy and clean-living environment.

Respite Services

  • Relief for members’ caregivers because caring for others starts with caring for yourself.
  • Respite can be provided in the member’s home or, in some instances, outside the home
