About the program:

Given the nationwide move from fee-for-service to value-based care, healthcare is now being evaluated by quality of care and outcomes.  

This move necessitates addressing the social determinants of health, which are the conditions in which people are born, grow, live, work and age. They include factors like socioeconomic status, education, neighborhood and physical environment, employment, and social support networks, as well as access to health care.

With over 24 years of experience assisting individuals to recover and age in the home, the team at J & M Homecare has a robust understanding of and access to the resources and support systems needed to flourish.

By providing Client Assessments, Care Coordination and Extended Care Management services and strategic resource support, as well as private duty caregiving, J & M Homecare is able deliver the value-based resources and support needed to assist individuals and families to positively affect health outcomes.
